Small Rural Hospital Transition (SRHT) Project Technical Assistance - National Rural Health Resource Center (Deadline: 12/31/2020)Project supports small rural hospitals nationally in transitioning to value-based systems by providing technical assistance (TA) through educational trainings and comprehensive onsite consultations. Eight hospitals are selected each year to receive on-site consultation projects. SRHT selected hospitals may choose to participate in either a financial operational assessment (FOA) or a quality improvement (QI) and transition of care project.
